Access Care Co-ordinator
Our access co-ordinator Laura is working with the whole board to improve information for patients, patients satisfaction and ensuring we are providing as much clinical time to suit patients as possible. Ensuring all clinics are accessible and evenly split between the 4 Surgeries and are filled with appropriate problems that can be dealt with in the first instance.
Laura also works most of our evening clinics as a Receptionist and audits all patient outcomes highlighting anything urgent to the relevant Surgeries
Our individual GP care coordinators work closely with our lists ensuring that they are up to date and all patients have the care, help and support in place as much as possible. They often liaise with outside agencies where needed to ensure patients records are kept up to date with he correct information
