Social prescribers work in partnership with GP Surgeries to help access appropriate support and care in the community. The Social prescribers can help with a range of difficulties people may face from their health to mental health and domestic abuse
Any type of health care can refer to an SP after assessing their individual and initial issue. SP’s are able to signpost patients to mental health support in the community which meets the patient requirements. Help with bereavement and refer where necessary.
Social prescribers can also health with weight management. Giving further advise of eating habits and advising or referring for physical activity. These things can often lead to further problems with mental health and making patients feel anxious and or alone with their problems.
How do Social Prescribers help the NHS?
They can help to assess and direct patients over the age of 18 once referred by a clinician. This helps to save clinical time with more acute illness and also reduces the amount of medicines by enabling patients to take control of their own physical and mental health
